Scheduling an appoinment is easy.
Things to Know prior to scheduling
1) A valid credit card is required to be on file at all times. Failure to do so may result in cancellation of the appointment.
2) Once you have requested an appointment, you will receive an email confirming or rejecting the appointment based on availability.
3) If your appointment has been accepted, you will receive an email regarding our portal and paperwork that is required to be completed 24 hours prior to your appointment. If this paperwork is not completed 24 hours prior, your appointment will have to be rescheduled.
4) You can view the Informed Consent and Credit Card Consents below prior to requesting an appointment.
